We support your right to choice and control in how your personal information is handled and ensure that all information is managed securely, responsibly, and transparently.

Collection of Personal Information

We collect personal information that is reasonably necessary to provide our services and supports. This may include your name, contact details, date of birth, and information about your needs, goals, and supports. In some circumstances, we may also collect sensitive information, such as health-related information, where it is required to deliver appropriate services.

Information is collected directly from you wherever possible. In some cases, it may also be collected from your family members, carers, authorised representatives, or other service providers, provided that you have given consent or where collection is otherwise permitted or required by law. We will explain why we are collecting your information and how it will be used so that you can make informed decisions.


Use of Personal Information

Your personal information is used to deliver safe, appropriate, and person-centred supports. This includes developing and reviewing your services, communicating with you and those you nominate, and meeting our legal, regulatory, and NDIS obligations. We only use your information for the purpose for which it was collected, or for a related purpose that you would reasonably expect.


Disclosure of Personal Information

We may disclose your personal information with your consent, including to your nominated representatives or other service providers involved in your supports. Information may also be disclosed where it is required or authorised by law, or where it is necessary to prevent or lessen a serious threat to health or safety. We do not sell or trade personal information.


Storage and Security

We take reasonable steps to protect personal information from misuse, loss, unauthorised access, modification, or disclosure. This includes maintaining secure physical and electronic systems, implementing access controls based on roles and responsibilities, and ensuring staff are trained in privacy and confidentiality obligations. Personal information is stored only for as long as necessary and is securely archived or disposed of in accordance with legal and operational requirements.


Access and Correction

You have the right to access the personal information we hold about you and to request corrections if that information is inaccurate, incomplete, or out of date. Requests for access or correction will be managed in a timely and respectful manner.

We will seek your consent when collecting, using, or sharing your personal information unless otherwise permitted or required by law. You may withdraw your consent at any time, subject to legal or operational limitations. If you are not satisfied with the outcome, you may contact the Office of the Australian Information Commissioner at www.oaic.gov.au or by phone on 1300 363 992. You may also contact the NDIS Quality and Safeguards Commission if your concern relates to NDIS supports or services.


Responsibilities

All staff, volunteers, and representatives are required to maintain confidentiality, respect the rights and dignity of individuals, and handle personal information in accordance with this policy, relevant legislation, and the NDIS Code of Conduct.
